A full syringe of lip filler is $500 at Serene. Here’s what 1 mL does, how long it lasts, and how we choose between Juvéderm Volbella XC and Ultra XC.
Lip filler can soften unevenly over a few months. A physician explains why, shows a real Volbella XC touch-up, and why follow-up care matters, in Hudson, OH and Barboursville, WV.

A few months ago, one of our patients had her lips treated with Juvéderm Volbella XC here at Serene. She loved the result. Then, about three months later, she came back with a concern: when she smiled, she could see a small dip in the center of her upper lip, and a slight flat spot along the lower edge of the right side of her upper lip. At rest, it was hard to notice. In a smile, it stood out.
She wasn’t sure what to expect. Did she need a whole new syringe? Had something gone wrong?
Neither. And the way we handled it says a lot about how we care for patients at Serene Med Spa in Hudson, Ohio and Barboursville, West Virginia: we don’t treat and disappear. If something changes, we want you back in the chair so we can look at it together.
After examining her lips at rest and while smiling, I placed a small amount of Volbella XC in just the two areas that had softened:
The goal wasn’t more volume. It was balance, restoring an even shape so her lips look natural from every angle. No full re-treatment, no “starting over.”


I asked her to come back in two weeks. By then, most of the early swelling has settled, and we can check the result at rest and in a full smile. We’ll update this post with her follow-up photos.
Hyaluronic acid (HA) fillers like Volbella are designed to be temporary. Your body gradually breaks them down, and that process isn’t always perfectly even. Here’s what the research tells us:
To be clear, no published study has measured exactly why one part of a single lip softens before another. Movement, how the filler settled and each person’s own tissue are the most likely explanations. What we do know is that it happens, and that it’s fixable.
Longer for some people than others. Volbella XC is FDA-approved for lip augmentation and for lines around the mouth in adults over 21. In its U.S. clinical trial, about 62% of patients still had improved lip fullness one year later. A European study found 48% were still responding at 12 months.
A 2021 analysis that pooled lip filler studies found that about 91% of patients still had a response at 2 months, 74% at 6 months and 46% at 12 months. In other words, some patients see changes well before the one-year mark, which is exactly what happened here.
When you smile, your upper lip stretches and thins. Small differences in volume that are invisible at rest can suddenly show. That’s why a lip evaluation shouldn’t stop at a still photo. A 2025 expert framework on natural-looking lip filler recommends checking the lips in motion, not just at rest. At Serene, I look at your lips relaxed, talking and smiling before deciding where filler is needed.
Touch-ups aren’t a sign that something went wrong. They’re built into how these fillers were studied:
That’s the idea behind a targeted touch-up: add only what’s needed, where it’s needed. Many touch-ups use far less than a full syringe. If you do need more, our half-syringe option is $300 and a full syringe is $500, at both locations.
It can happen. Hyaluronic acid filler breaks down naturally, and it doesn’t always soften at the same rate everywhere, especially in a part of the face that moves as much as the lips.
Not necessarily. Often a small, targeted amount in the areas that softened is enough to restore an even shape.
Smiling stretches and thins the upper lip, so small volume differences show more. That’s why we evaluate lips at rest and in motion.

We usually see patients about two weeks after treatment, once early swelling has settled, and anytime you have a concern.
