For years, Medicare didn’t cover medications for weight loss. That changed on July 1, 2026. Under the new Medicare GLP-1 Bridge program, eligible patients can get select GLP-1 medications for $50 a month.
Many patients in Barboursville, Huntington, Milton, Hurricane, Proctorville and Ashland are asking us what this means for them. Here’s a plain-language guide, plus the one thing every patient over 65 should plan for before starting.
What is the Medicare GLP-1 Bridge program?
It’s a temporary Medicare benefit that runs from July 1, 2026 through December 31, 2027. Eligible patients pay a $50 copay for a one-month supply of a covered GLP-1 weight-loss medication.
- You need a Medicare Part D or Medicare Advantage plan to use it.
- Your prescriber must submit a prior authorization. Once it’s approved, your pharmacy is notified.
- Lifestyle counseling is part of the program. Patients receive guidance on nutrition and activity while taking the medication.
- The $50 copay doesn’t count toward your Part D deductible or out-of-pocket limit, and Extra Help can’t lower it.
Who qualifies?
Eligibility is based on your BMI and your health conditions. You may qualify if you have:
- A BMI of 35 or higher, or
- A BMI of 30 or higher with heart failure, uncontrolled high blood pressure or kidney disease, or
- A BMI of 27 or higher with a previous stroke or heart attack, prediabetes or peripheral artery disease.
As a kidney specialist, I’m glad to see kidney disease on that list. Excess weight puts real strain on the kidneys and on blood pressure. Kidney patients also need extra care with protein intake, hydration and side effects like nausea, so treatment should always be physician-supervised.
The question the scale can’t answer
Two people can each lose 20 pounds on a GLP-1. One loses mostly fat. The other loses a lot of muscle. The scale reports both as success.
In large clinical trials, lean mass made up roughly a quarter to about 40% of the weight people lost on GLP-1 medications. For adults 65 and older, that matters. Muscle protects you from falls and fractures, keeps you steady on your feet and helps you stay independent. BMI may qualify you for treatment, but it can’t tell you whether treatment is going well.
How to protect your strength
- Make protein a priority. Appetite drops on a GLP-1, and protein is often the first thing to slip. Many experts suggest 1.2 to 1.6 grams per kilogram of body weight per day during weight loss. If you have kidney disease, set your target with your doctor first.
- Do simple strength work three times a week. Chair stands, resistance bands and light weights all help. Add walking most days if you can.
- Go slow. Gradual dose increases are easier on your stomach and on your muscles.
- Watch for warning signs. Weakness, dizziness, trouble getting out of a chair or a fall are reasons to call your provider right away.
- Keep your follow-up visits. Regular check-ins let your physician adjust the dose, nutrition and activity plan as you go.
